Medical Social Services helps the patient and their families cope with the personal and emotional impact of illness or disability. The Social Worker will also help locate financial resources and community assistance such as home meal delivery or support group.
Our medical social workers will:
- Coordinate with community resources to assist the patient.
- Contact support groups and maintain a channel to the patient’s needs.
- Assist the patient in dealing with personal relationship problems with their family.
- Help patients who face disabilities or any life-threatening disease to plan their care.
- Assist in planning their finance such as housing, unemployment, or inadequate nourishment.
- Assist patients through counseling in coping and adjusting in their difficulties (such a new prosthetic or wheelchair-bound).
- Identify eligibility for services and benefits.
- Advocate for patient rights if there is a sign of abuse in their homes.
